  • Violation of Code: [.04(6)(g) ] Ready-to-eat potentially hazardous foods in the reachin coolers were not date marked: meats, cheeses, lasagna, tiramisu. Foods were date marked at the time of inspection. Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety food) 1) prepared and held in a food establishment for more than 24 hours, AND/OR 2) prepared and packaged by a food processing plant, shall be clearly marked, at the time the original container is opened in a food service establishment and if the food is held for more than 24 hours, to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, which is including the day the original container is opened, a maximum of seven (7) calendar days after the original container is opened, or the useby/ sell by/ expiration date, whichever comes sooner, if the food is maintained at 41°F (5°C) or below.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(3)(l) ] New Zealand green mussel boxes/tags not kept for 90 days. Shellstock boxes/tags shall be kept for 90 days.  New Violation.
  • Violation of Code: [.04(3)(k) ] New Zealand green mussels taken out of the original box & stored in a pan in the cold top cooler. PIC voluntarily discarded mussels. Molluscan shellfish may not be removed from the container in which they are received other than immediately before sale or preparation for service.  Corrected On-Site.  New Violation.
  • Remarks  Violation of Code: [.04(4)(i) ] Raw New Zealand green mussels & raw shrimp stored in undrained ice in the cold top cooler. Unpackaged food may not be stored in direct contact with undrained ice.  New Violation.
  • Violation of Code: [.05(6)(a) ] Peeling coating/rust on upright cooler shelves. Broken wires on fry basket. Maintain equipment/utensils in good repair.  New Violation.
  • Violation of Code: [.05(7)(d) ] Grease accumulated inside the fryer cabinet. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.  New Violation.
  • Violation of Code: [.05(7)(a)2,3 ] Sticker residue on clean food containers. Dust/buildup accumulated on the dish storage shelves. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.  New Violation.
  • Unwashed produce stored above ready-to-eat foods in the walkin cooler. Raw animal foods stored above ready-to-eat foods in the walkin freezer. Food shall be protected from contamination by: 1) storing unwashed produce below/away from ready-to-eat foods, washed produce; 2) storing raw animal foods below ready-to-eat foods. Food storage chart provided.
  • Missing drain plug on dumpster. Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place.

  • Remarks: * dish machine not registering chlorine until water changed out. Machine may be getting to hot and making the chlorine evaporate. Temp of machine reachin 152.2. Cook was checking dish machine at beginning of inspection (not washing dishes at time of inspection) *CFSM certificate not on site at time of inspection. Still within the 90 days of expiration to have CFSM certificate posted. Make sure original copy is posted. The CFSM certificate can only be used for one facility.

  • Violation of Code: [.03(3)(a)-(d) ] The original copy of the certified food safety manager (CFSM) certificate could not be found during inspection. The original copy of the CFSM certificate needs to be on site and posted. Please locate original and post.  New Violation.
  • Violation of Code: [.03(4)(a)-(f) ] Person in charge was unable to locate the employee health policy. Person(s) in charge need to have knowledge of the employee health policy which included 5 symptoms, 5 diseases, and what to do in the event employee exhibit or report symptoms or diseases. Handout of the employee health policy was given during inspection and discussed with the person in charge.  Corrected On-Site.  New Violation.
  • Violation of Code: [.03(5)(i) ] Employee's working with pizza dough had a hat on, but did not have hair properly restrained(hair handing down in face). Employees with hair longer than 1/2 inch (scalp or beard) must wear proper hair restraint. Employee stopped and pulled hair back under hat.  Corrected On-Site.  New Violation.
  • Violation of Code: [.07(5)(f) ] Wet mop was left in bucket. Mops need to hung to dry. Employee hung mop during inspection.  Corrected On-Site.  New Violation.

Person in charge indicated that cheese had been out less than 4 hours. Cheese marked with proper time.   • Remarks - NOTE: Dish washing machine was malfunctioning during inspection. Employees were using the 3 compartment sink until the dish machine worked properly. Dish machine was working at the end of the inspection.

  • Violation of Code: [.07(5)(f) ] Wet mops stored on floor in back room across from dish washing machine. Wet Mops should be hung to dry after use.  New Violation.

Observed employee cutting raw chicken then touch the walk-in cooler handle without changing gloves. Observed raw chicken store behind raw beef and pooled eggs in the prep cooler. Had employee remove gloves, rewashed dishes and sanitized equipment touched. Had PIC move chicken to the front of the prep cooler. Food should be kept separated. When handling raw chicken employees should change gloves and wash hands before touching clean equipment and dishes.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(k) ] In-use utensils for hot-held potentially hazardous foods not stored in a container of 135oF water. In use-utensils stored in water at 111oF and 79oF. Units turned on to heat water up to 135oF or above. In use utensils stored in water should be stored in 135oF or above.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(n) ] Employees using single use gloves for more than on task. Employees handling raw chicken then clean equipment and dishes with same pair of gloves. Had employees remove gloves. Gloves should be used for a single task.  Corrected On-Site.  New Violation.

  • Violation of Code: [.03(5)(j)1&2 ] Employee drinks in approved cups were stored on food prep counter near vegetable sink and on counter across from where chicken is breaded. Employee drinks need to be in disposable cup with a lid and straw, and also need to be stored away from food, food prep surface, single service items, utensil, and equipment. Cups were moved.  Corrected On-Site.  New Violation.
  • Violation of Code: [.03(5)(g) ] Observed an employee wearing 2 bracelet while working with food. The only jewelry employee can wear while working with food is a plain ring. Employee removed bracelet washed hands and put on new gloves.  Corrected On-Site.  New Violation.
