  • Violation of Code: [.04(6)(d) ] Observed pizza sauce cooling for over 4 hours still above 41F. Person in charge discarded sauce. Advised person in charge must cool potentially hazardous ready to eat food below 41F with in 4hrs.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(e) ] Pizza sauce cooling over 4 hours still above 41F. PIC discarded item. Advised PIC must properly cool PHF with in 4hours.  Corrected On-Site.  New Violation.

Observations and corrective actions:

  • Violation of Code: [.05(7)(a)1 ] Food contact surfaces not cleaned to sight and touch. Food contact surface (dining room tables) not cleaned adequately.
  • Microwave oven, utensils, containers, knife sharpener not being cleaned properly. Utensils being used and then put back in clean container with food on them. Items were cleaned and sanitized during inspection.
  • Dining room tables not being cleaned by correct process. Tables being cleaned with soapy water and wiping cloth. Tables must be cleaned and sanitized by one of two methods. 1. 1 Step Method = Sanitizer in spray bottle (100 ppm chlorine or Mfg approved ppm level for quat) with DISPOSABLE paper towels OR 2. 2 Step Method = 1 wiping cloth from a bucket of hot soapy water or sanitizer to remove food debris from table then 1 wiping cloth from a bucket of sanitizer at approved level.
  • Must clean food contact surfaces to sight and touch and frequently.  Corrected On-Site.  New Violation.
  • Remarks: Violation of Code: [.04(4)(q) ] Food not protected from contamination while on bottom food prep shelf. Flour and breading left uncovered on bottom shelf. Flour had a spray can stored on top of the food product. Can was removed and food items covered. Must store food so that it is protected from contamination.  Corrected On-Site.  New Violation.
  • Violation of Code: [.03(5)(i) ] Employee not using an effective means of restraint for hair, beard, or mustache longer than ½ inch.
  • Employees working in food prep area not wearing any type of hair restraint. No hair restraints on site. Please purchase proper hair restraints; hats, hair nets, etc.
  • Must restrain hair that is longer than 1/2 inch while working in food prep/dish wash areas.  New Violation.
  • Violation of Code: [.05(3)(h),(i) ] Quat test strips not available for testing the sanitizer level for the dish sink. Please purchase quat test strips. Must have proper testing device for all chemical sanitizers. Facility has chlorine test strips on-site. Facility also had pool test strip and asked that they remove those from the food prep area since they cannot be used for the dishwasher.  New Violation.
  • Violation of Code: [.05(3)(d) ] Vent hoods unclean with grease build-up. Please clean vent hoods often enough to prevent grease build-up that can potentially drip down on food in the prep area.  New Violation.
  • Note: Facility was permitted without a dishwasher. Dishwasher was added right after opening inspection. Dishwasher was not noted on previous inspections. Conducted an on-site field test to determine if there is enough hot water during peak demand. The facility passed this evaluation.
  • Note: New menus were printed with the correct disclosure and reminder statement for oysters, salmon and steak.
  • Note: Facility just added asterisk to salmon on menu to be offered as undercooked. Facility needs a parasite destruction letter/farm raised and pellet fed letter for the salmon. PIC stated that salmon is ordered fresh so the letter needs to state that the salmon was farm raised in net pens and pellet fed to be parasite free. Must keep this letter on-site. Provide a letter from every supplier that you receive salmon from. This will be marked as a violation if proper documentation is not available on next inspection (9 points)
  • Note: 2 compartment sink must be used for fruits and vegetables. Meats must be thawed in the 3 compartment sink when dishwashing is not in process and then sanitized thoroughly before beginning dishwashing. Facility was not washing fruits and vegetables or thawing meats during the inspection but after questioning, determined that sinks were being used incorrectly. This will be marked as a violation on the next inspection if observed to be using the wrong sinks for these processes (3 points)
  • Note: CFSM certificate expires 8/16/13, be sure they re-test before it expires.
  • No raw whole oysters on-site during inspection. Shucked oysters on-site in proper container with proper identification and approved.

  • Violation of Code: [.03(5)(b) ] Observed employees washing hands in cold water only. Employee rewashed hands. Advised person in charge hands must be washed in warm water.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(m) ] Observed wet wiping clothes stored on counter. Person in charge placed clothes into buckets of sanitizer. Advised PIC must store wet wiping clothes in buckets of sanitizer.  Corrected On-Site.  New Violation.
  • Violation of Code: [.10(2)(g) ] Observed an older inspection report hanging in public view instead of the latest. PIC placed this report up. Advised PIC must place the latest inspection report no matter the score.  Corrected On-Site.  New Violation.

 

Holy Smokes Barbecue

  • Violation of Code: [.07(5)(a),(b) ] Floors not clean in storage room. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.  Repeat Violation.

  • Violation of Code: [.07(5)(k) ] At time of inspection, observed several live and dead roaches around cove base of wall in kitchen area. Person in charge was able to verify that facility has monthly pest service from a licensed pest control company. Person in charge called company during inspection and is going to have an additional treatment tomorrow, May 31. Must ensure pest are controlled.  New Violation.

  • Violation of Code: [.05(7)(a)1 ] Meat slicer had dried food debris. Slicer was cleaned at time of inspection. Food contact surfaces so be kept clean to sight and touch.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(10)(a) ] Large containers "wet-nested". After cleaning and sanitizing equipment should be air dried before stacking.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(7)(a)2,3 ] Gaskets on front beer cooler contained black residue. PIC cleaned gaskets. Nonfood contact surfaces should be kept clean.  Corrected On-Site.  New Violation.
  • Violation of Code: [.07(5)(f) ] Mop in mop sink, not hung to dry. Person in charge hung the mop. Mops must be hung to dry.  Corrected On-Site.  Repeat Violation.
