  • Remarks: Hot water restored. PERMIT REINSTATED. Additional routine in 45-60 days from previous routine inspection. IF FACLILITY MAKES A U ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Employees signed up for food safety class. Hours of operation pending.

  • Violation of Code: [.03(5)(j)1&2 ] Employee drink in prep area in a non disposable cup without a lid. Cup removed and sent to dishwashing. All employee drinks must be in a disposable cup with a lid and a straw.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(f) ] Multiple items in both prep coolers above 41F. Foods were discarded. All potentially hazardous foods must be cold held at 41F or below.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(g) ] Multiple potentially hazardous items in the walk in not date marked, including soups, tomatoes and sausage rolls. Items were marked with the date they were prepared. All potentially hazardous foods must be date marked and product discarded within 7 days.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(6)(h) ] Chili prepared 1-7-13, alfredo sauce with a preparation date of 12-21- 12 still in coolers. Food was discarded. All potentially hazardous foods must be date marked and product discarded within 7 days.  Corrected On-Site.  New Violation.
  • Remarks: Date marking is a repeat violation. A third repeat on inspection will result in closure of the facility.

  • Violation of Code: [.05(6)(r) ] PINT milk gallon-like containers being used for flavorings (pouring flavoring into jug). These containers are similar to milk jugs (but smaller) and are not easily cleanable. Food storage containers must be smooth, durable, easily cleanable, & nonabsorbent.  New Violation.
  • Violation of Code: [.05(7)(d) ] Build up on gaskets, wire shelf above prep cooler, interior of reach in coolers. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.  New Violation.
  • Violation of Code: [.06(2)(c) ] No warm water at handwashing sink in the back kitchen. Must supply warm water at 100 deg. F to all handwashing sinks.  New Violation.
  • Violation of Code: [.06(5)(o),(p) ] Leaf litter surrounding dumpster. Keep area clean.  New Violation.
  • Violation of Code: [.07(3)(f) ] Light not working in freezer. Repair light to freezer.  New Violation.
  • Remarks: Battered chicken strips are uncooked as stated on box. Discussed with owner. Possible this changed last fall. Will need to update facility to Risk Type 2.  Ensure children do not wander throughout kitchen.

  • Violation of Code: [.05(6)(n) ] NO SANITIZER WAS SHOWING UP ON THE CHLORINE TEST PAPER. CHANGE OUT THE SANITIZER AND REPRIME IT. HAVE THE SANITIZER DISPENSING SYSTEM FIXED, ADJUSTED, AND REPLACED IF NEEDED TO PROVIDE AT LEAST 50 PPM AVAILABLE CHLORINE TO THE FINAL RINSE OF THE WAREWASHING MACHINE.  New Violation.
  • Violation of Code: [.04(6)(f) ] BUTTER WAS 47.3 DEGREES F IN THE WALK-IN COOLER. POLENTA WAS 46.8 DEGREES F. CHICKEN STOCK IN THE PREP REFRIGERATOR ACROSS FROM RANGE WAS 46.1 AND 47.6 DEGREES F. TOMATO SAUCE WAS 49.0 DEGREES F. DISCARD ALL THE POTENTIALLY HAZARDOUS FOODS THAT HAVE BEEN ABOVE 41 DEGREES F FOR MORE THAN FOUR HOURS.  New Violation.
  • Violation of Code: [.05(6)(a) ] WALK-IN COOLER UNIT WAS ICED UP. REPAIR UNIT TO HOLD FOOD AT 41 DEGREES F OR LESS. FOOD IN THE WALK-IN COOLER WAS 46.8 AND 47.3 DEGREES F. HAVE THE COMPRESSOR COME ON AT 40 DEGREES F AND CUT OFF AT 34 DEGREES F. HAVE THE FOOD PREP REFRIGERATOR AT THE RANGE REPAIRED ALSO. THE AIR TEMPERATURE IN THE PREP REFRIGERATOR WAS 45.9 DEGREES F.  New Violation.
  • Violation of Code: [.05(6)(p) ] SANITIZER DISPENSER FOR WAREWASHING MACHINE WAS NOT PROVIDING AT LEAST 50 PPM AVAILABLE CHLORINE. REPAIR OR REPLACE THE SANITIZER DISPENSER.  New Violation.

  • Violation of Code: [.05(3)(g) ] Thermometer for measuring the internal temperature of thin-massed foods not provided. Thermometer for measuring the internal temperature of thin-massed foods must be provided.  Repeat Violation.
  • Violation of Code: [.06(2)(c) ] Warm water not supplied to restroom handwashing sinks. Must supply warm water at 100 deg. F to all handwashing sinks.  Repeat Violation.
  • Remarks: At time of inspection, multiple Papa Johns pizzas were delivered. Multiple Papa Jonhs pizza boxes were found in the trash in the back office/storage/warewashing area. Pizza by the box on menu board for sale. Screen menu also has pizza by the box and pizza by the slice listed as an option. A customer was overheard asking if facility has pizza by the slice today. Also, the hot holding cabinet in the back room was warm to touch but it had a notice "Employees Only" posted. Manager stated that they are purchasing pizza by the box upon request from customer. When they re-sell, they re-sell the entire box to client. Facility is not to resell pizza by the box or slice from Papa Johns or Little Caesars or any other facility that does not hold a proper permit from GA Department of Agriculture. Please contact Jason, QA for Newton, Rockdale & Gwinnett Counties for further clarification & to find a resolution to issue. A citation will be issued if there is no resolution to issue & facility continues to resell pizza from facilities that do not hold proper permit from Department of Agriculture.

  • Violation of Code: [.03(5)(g) ] Employee wearing multiple bracelets while working with food. Had employee remove bracelets at time of inspection. Except for a plain ring such as a wedding band, while preparing food, food employees may not wear jewelry including medical information jewelry on their arms and hands.  Corrected On-Site.  New Violation.
  • Violation of Code: [.02(1)(d) ] Most recent food service permit not posted. Permit posted expired in August 2012. Must post the most recent food service permit for public view.  Repeat Violation.
  • Violation of Code: [.07(5)(a),(b) ] Floors not clean in storage room. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.  New Violation.
  • Remarks: Dining counters/table-tops shall be cleaned and sanitized routinely after removing all soiled tableware and food trays shall be cleaned and sanitized after each use by one of the following methods or other method approved by the Health Authority: 1. A two step method may be used in which one cloth, rinsed in sanitizing solution is used to clean food debris from the surface and a second cloth in separate sanitizing solution is used to rinse. Or 2. Sanitizing solution is sprayed onto the surface and the surface is then wiped clean with a disposable towel. DO NOT USE GLASS CLEANER TO CLEAN TABLES.
