Observations and corrective actions:

  • Violation of Code: [.03(5)(c) ] No hand washing before engaging in food preparation. PIC was outside of the facility, went to the pizza oven & then touched the pizza with her unclean hand. Training provided; pizza was discarded & PIC washed her hands. Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles.  Corrected On-Site.  New Violation.
  • Violation of Code: [.03(4)(a)-(f) ] Person in Charge (PIC) unable to convey knowledge of the employee health policy (symptoms, illnesses, reporting requirements, exclusion/restriction requirements). Training provided on the employee health policy at the time of inspection. All employees shall know the employee health policy (symptoms, illnesses, reporting requirements, exclusion/restriction requirements).  Corrected On-Site.  New Violation.
  • Violation of Code: [.07(3)(b) ] Empty paper towel dispenser by the pizza oven. Paper towel provided at the time of inspection. Ensure hand sinks are stocked with paper towels at all times.  Corrected On-Site.  New Violation.
  • Violation of Code: [.03(5)(i) ] PIC was not wearing an adequate hair restraint while preparing pizzas; only part of her hand was tied back. Employee put on a hat at the time of inspection. Employees preparing and/or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch (1/2").  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(k) ] Scoop handles were stored in direct contact with food in the cold top cooler. While not in use, food preparation/dispensing utensils shall be stored with their handles above the top of the food and the container.  Repeat Violation.

  • Violation of Code: [.03(5)(j)1&2 ] Employee's water bottle stored on the prep table, above food & next to utensils. Water bottle removed at the time of inspection. Employee drink cup without a straw, stored next to napkins. Drink moved below napkins. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas, clean linens, and food storage areas. Drinks shall be in a single service beverage cup with a secure lid and straw.  Corrected On-Site.  New Violation.
  • Violation of Code: [.06(2)(o) ] A serving tray & sponge were stored in the kitchen hand sink. Tray & sponge removed at the time of inspection. A handwashing sink shall be maintained so that it is accessible at all times for employee use. A handwashing facility may not be used for purposes other than handwashing. **IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT INSPECTION, THE PERMIT MAY BE SUSPENDED FOR 3RD REPEAT VIOLATION.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Raw eggs stored above sauces & vegetables in the upright cooler. Unwashed produce stored above washed produce & bread in the walkin cooler. Raw seafood stored above an open package of corn in the walkin freezer. Raw beef stored above raw shrimp, raw pork, ice cream, fried ice cream in the walkin freezer. At the time of inspection, raw foods moved below ready-to-eat foods & raw animal foods were separated by minimum cooking temperatures. Food shall be protected from cross contamination by: 1) storing raw animal foods below/away from produce/cooked/ready-to-eat foods; 2) storing/ separating raw animal foods by the minimum cooking temperature; 3) storing unwashed produce below washed produce/ready-to-eat foods.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(g) ] Ready-to-eat potentially hazardous foods not date marked: flan, imitation crab meat, queso. Foods were date marked at the time of inspection. Refrigerated, ready-to-eat, potentially hazardous food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, which is including the day of preparation a maximum of seven (7) calendar days from the day the food is prepared if the food is maintained at 41°F (5°C) or below.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(h) ] Ready-to-eat potentially hazardous foods held past the date mark/use by date: Cheese 2/12/13, cooked poblano peppers 2/15/13, ham 2/16. Foods were discarded at the time of inspection. A food that requires datemarking shall be discarded if it exceeds 7 days/use by date.  Corrected On-Site.  New Violation.
  • Violation of Code: [.07(6)(b) ] Unlabeled bleach spray bottle & sanitizer bucket. Containers were labeled at time of inspection. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.  Corrected On-Site.  New Violation.
  • Violation of Code: [.07(6)(c) ] Spray bottle of spic & span stored on the shelves above single-use utensils. Bottle was removed at the time of inspection. Poisonous or toxic materials shall be stored so they can not contaminate food, equipment, utensils, linens, and single-service and single-use articles by: 1) Separating the poisonous or toxic materials by spacing or partitioning; and 2. Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(6)(a) ] Ice buildup in the walkin freezer (walls, floors, interior surface of door/doorframe), shelves & on the food packaging. Maintain equipment in good repair.  New Violation.

  • Violation of Code: [.05(3)(g) ] No small-diameter probe thermometer available in the facility. A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.  Repeat Violation.
  • Violation of Code: [.03(5)(i) ] Two employees not using an effective means of restraint for hair. Employees preparing and/or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch (1/2").  Repeat Violation.
  • Violation of Code: [.05(1)(a) ] Facility is using printed paper towel to cover biscuits & to absorb grease from chicken, bacon. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be safe, durable, corrosion-resistant, and nonabsorbent.  New Violation.
  • Violation of Code: [.06(5)(b) ] Grease bin & trash cans stored on grass. An outdoor storage surface for refuse, recyclables, and returnables shall be constructed of nonabsorbent material such as concrete or asphalt and shall be smooth, durable, and sloped to drain.  Repeat Violation.
  • Violation of Code: [.07(5)(d) ] Dust/grease buildup on the hood/filters. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.  New Violation.
  • Violation of Code: [.07(4)(b) ] Makeup, purse, hairbrush, phone stored on the service line shelf above single-use utensils & soda syrups. Personal items moved below food & utensils. Personal items shall be stored separate from food, food areas, clean linens & utensils/equipment storage.  Corrected On-Site.  Repeat Violation.
  • Facility is now open on Friday nights from 4:30pm - 6:30pm. All raw animal foods are fully cooked in the kitchen & hot held in the steam table. Raw animal foods are not offered raw/undercooked.
  • No warm water in the customer restroom (73F); all other handsink had warm water >100F. A handwashing sink shall be equipped to provide tempered water at a temperature of at least 100ºF (38ºC) through a mixing valve or combination faucet.
  • Facility is using Lysol wipes to clean the dining tables between customers. Dining tables shall be cleaned and sanitized using sanitizer at an approved concentration. (i) A two step method may be used in which one cloth, rinsed in sanitizing solution is used to clean food debris from the surface and a second cloth in separate sanitizing solution is used to rinse; or (ii) Sanitizing solution is sprayed onto the surface and the surface is then wiped clean with a disposable towel.

  • Violation of Code: [.06(2)(c) ] Water temperature at restroom handwashing sink at 90 deg. F. Water temperature must be at 100 deg. F.  New Violation.
  • Remarks: This facility uses raw unpasteurized eggs & makes chicken salad in house (raw chicken cooked in house). This a Risk Type 2 facility. Cream cheese & heavy whipping cream are used in cakes that are made in house.

  • Violation of Code: [.04(6)(i) ] Pizza sauce not documented with the time when removed from temperature control. Had the manager document the pizza sauce from 10-2. Facility must document when the pizza sauce is removed from temperature control as per written procedures (posted on wall). IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Corrected On-Site.  New Violation.
  • Violation of Code: [.07(6)(c) ] Degreaser stored on pizza box. Degreaser relocated to an approved location. Chemicals must be stored where they will not contaminate food, food service equipment, or supplies. IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Corrected On-Site.  New Violation.
  • Violation of Code: [.06(2)(r) ] Handle for hot water broken - loose. Plumbing system must be maintained.  New Violation.
  • Remarks: Facility date-marks mozzarella, American cheese, feta cheese for 10 days once it is opened. These cheeses may not be date marked for more than 7 days while under refrigeration. Manager stated that cheeses generally don't last for more than 7 days.
  • IF FACILITY MAKES A U ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED. Additional routine inspection due in 30-60 days. Dough containers and equipment by back door need to be elevated off the floor 6 inches.
