Observations and corrective actions:

  • Violation of Code: [.03(5)(j)1&2 ] Water bottle with lid and no straw on table in kitchen. Monster drink on same table in kitchen. Both drinks re-located to the office. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. Risk Control Plan paperwork provided. The certified food safety manager is to complete, fax or email to office. Contact information provided. THE FOOD SERVICE PERMIT MAY BE SUSPENDED IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(6)(j) ] Italian sausage and pepper sausage heated to 155, 153 deg. F in oven. Product ran through oven again & it reached a temperature above 165 deg. F. Must heat up to 165 deg. F as per blanket variance for bare hand contact with ready to eat foods in pizza establishments. IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.05(10)(a) ] Containers for food stacked wet. After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.  New Violation.
  • Violation of Code: [.06(2)(r) ] Lever/handle to hot water loose at both handwashing sinks. When set, it moves & increases the water to a temperature that is too hot for proper handwashing. Plumbing system must be maintained.  Repeat Violation.
  • Violation of Code: [.07(2)(l) ] Door to employee restroom does not close entirely. Fix the latching mechanism on door (catching on frame preventing door from closing). Door to restrooms must be self-closing & tight-fitting.  Repeat Violation.

  • Violation of Code: [.04(6)(f) ] Observed potenitally hazardous foods(pico, corn salsa, sour cream, guacamole and diced tomatoes) cold holding above 41F. Person in charged discarded items. Advised PIC all PHF when cold held must be below 41F.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(4)(g) ] Observed fruit and vegetable sink being used to thaw chicken. PIC moved chicken and washed rinsed and sanitized sink. Advised PIC the fruit and vegetable sink is only for fruits and vegetables.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(6)(a) ] The prep cooler could not hold PHF at or below 41F. PIC placed iced baths on the bar to use until repairman can fix. Advised PIC must maintian equipment at all time to keep them working at safe levels.  Corrected On-Site.  New Violation.
