• Violation of Code: [.03(5)(j)1&2 ] Employee drink on prep table, no lid or straw. Employee drink discarded. Employee drinks must be in a disposable cup with a lid and a straw and stored to prevent contamination.  Corrected On-Site.  New Violation.
• Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Using tongs to grab raw chicken and then used tongs to grab raw vegetables. Then used tongs to grab raw beef then to grab raw chicken. Foods discarded and new tongs placed for different items. Food must be kept and handled in such a way as to prevent contamination.  Corrected On-Site.  New Violation.
• Violation of Code: [.04(6)(f) ] Items on steam table below 135F. Items discarded. All potentially hazardous foods must be hot held at 135F or above.  Corrected On-Site.  New Violation.
• Violation of Code: [.04(8)(b) ] Ham in prep cooler expired on 11-21-13. Ham discarded. All commercially prepacked manufactured must be discarded immediately after sell by/ expiration date.  Corrected On-Site.  New Violation.
• Violation of Code: [.10(2)(g) ] Current inspection form not posted. New inspection posted. Most current inspection must be posted within 15 feet of primary door, 5-7 feet in height and where customer can approach within 1 foot.  Corrected On-Site.  New Violation.
• Violation of Code: [.04(4)(n) ] Employee with gloves on washed dishes then returned to food prep area to begin prepping food without removing glovesHad employee remove gloves. Gloves are to be used for one purpose only.  Corrected On-Site.  New Violation.
• Violation of Code: [.07(3)(h) ] Employee jacket and hats in food prep area. Employee belonging removed to employee area. Employee must have a designated locker or area for personal belongings.  Corrected On-Site.  Repeat Violation.

• Violation of Code: [.03(5)(b) ] Employee washed hands then dried them with a non disposable towel. HAd employee rewash hands and counseled employee about only using disposable towels for drying.  Corrected On-Site.  New Violation.
• Violation of Code: [.06(2)(o) ] Used handsink to fill blender containers and to dump blender container. Reviewed procedures and that handsink is for handwashing only. Handwashing sinks can be used to wash hands only.  Corrected On-Site.  New Violation.
• Violation of Code: [.07(6)(b) ] Chemical cleaners in spray bottles with no identifying label. Bottles labeled. All working containers must be labeled with the common name of the product contained within.  Corrected On-Site.  New Violation.
• Violation of Code: [.07(6)(g) ] Chlorine level in dish sink above 150 ppm. Sanitizer water remade to proper levels. Chlorine sanitizers must be between 50-100ppm.  Corrected On-Site.  New Violation.
• Violation of Code: [.03(5)(f) ] Preparing smoothies with nail polish on fingernails. Employees may not wear artificial nails or nail polish unless wearing gloves when exposed to food.  New Violation.
• Violation of Code: [.03(5)(g) ] Employee wearing rings on her thumb while preparing smoothies. Employees may not wear jewelry on their hands or arms other than a plain wedding band.  New Violation.

• Violation of Code: [.04(6)(f) ] Several items holding in hot-hold unit at temperatures below 135oF. Items were reheated because they have only been in unit for 1 hour. Hot-held potentially hazardous foods should be maintained above 135oF; no time controls/documentation in place.  Corrected On-Site.  New Violation.
• Violation of Code: [.07(6)(g) ] Bleach mixed in with QUAT sanitizer at temperatures above 200 PPM. Sanitizer was discarded and new sanitizer provided. Advised PIC not to mix QUAT and Chlorine sanitizer. Sanitizer should be at manufactures concentration recommended levels.  Corrected On-Site.  New Violation.
