  • Violation of Code: [.05(7)(a)1 ] Interior of both ice machines with black slime like build up. Food contact surfaces must be clean to sight & touch. Address immediately. Will recheck in 3-10 days. IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Repeat Violation.
  • Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. Three stacks of cheese (American, cheddar, Swiss) above 41 deg. F, wings above 41 deg. F DISCARDED. Must hold at 41 deg. F or less. **Do not overstock containers.  Corrected On-Site.  New Violation. 
  • Violation of Code: [.04(6)(g) ] Refrigerated, ready-to-eat, potentially hazardous food prepared and held in the establishment for more than 24 hours (i.e., spinach dip, pasta, rice, shredded cheese) not clearly marked to indicate the date by which the food must be consumed, sold, or discarded. Had facility date mark food properly.   • Violation of Code: [.07(6)(b) ] Three chemical spray bottles not labeled. Had facility label chemicals. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Corrected On-Site.  Repeat Violation. 
  • Violation of Code: [.04(6)(e) ] Chicken wings cooling in covered container. Rice cooling in covered container. Both items transferred to thinner containers that were vented. Product dropped to 70 deg. F within 2 hrs after placed in freezer. Cooling shall be accomplished by using one or more of the following methods based on the type of food being cooled:
    (i) Placing the food in shallow pans; (ii) Separating the food into smaller or thinner portions;(iii) Using rapid cooling equipment;
    (iv) Stirring the food in a container placed in an ice water bath;
    (v) Using containers that facilitate heat transfer; (vi) Adding ice as an ingredient; or (vii) Other effective methods. 2. When placed in cooling or cold holding equipment, food containers in which food is being cooled shall be: (i) Arranged in the equipment to provide maximum heat transfer through the container walls; and (ii) Loosely covered, or uncovered if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food.  Corrected On-Site.  New Violation. 
  • Violation of Code: [.04(6)(c)] Raw chicken thawing in standing water (facet barely dripping). Raw chicken removed from water & placed in cooler. Must thaw via an approved method. See food code.  Corrected On-Site.  New Violation. 
  • Violation of Code: [.03(5)(h) ] Employee using apron to wipe hands after handling raw chicken (gloves were removed. Employee using apron to wipe face. The outer layer of clothing of all employees shall be clean. Food employee shall wear clean outer clothing to prevent contamination of food, equipment, utensils, linen, and single service and single-use articles.  New Violation. 
  • Violation of Code: [.05(10)(g)2 ] Unwrapped spoons, forks for self-service stored with food/lip surface towards customer. Had employee flip utensils properly. Knives, forks, and spoons that are not pre-wrapped shall be presented so that only the handles are touched by employees and by consumers if consumer self-service is provided.  Corrected On-Site.  New Violation. 
  • Violation of Code: [.05(7)(a)2,3 ] Grease build up on fryer baskets. Food label residue on exterior of multiple food storage containers. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.  New Violation. 
  • Violation of Code: [.07(5)(k) ] Rodent droppings in cabinet below soda machine. Flies and gnats in the kitchen/dishwash area. Must control pests in the establishment.  Repeat Violation. 
  • Went over proper cleaning of tables with staff.
  • IF FACILITY MAKES A U ON THE FOLLOW UP OR THE NEXT ROUTINE INSPECTION, THEN THE FACILITY WILL BE ASKED TO VOLUNARILY CLOSE, OR THE FOOD SERVICE PERMIT MAY BE SUSPENDED. 
  • Follow up in 3-10 days.

  • Violation of Code: [.06(2)(c) ] Water did not reach 100 deg. F at handwashing sink in women's restroom or men's restroom. A handwashing sink shall be equipped to provide tempered water at a temperature of at least 100ºF (38ºC) through a mixing valve or combination faucet.  New Violation. 
  • Violation of Code: [.06(5)(o),(p) ] Bags of trash on ground surround dumpster. Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. **Had facility to the right clean up their bags of trash that were on the ground.  Repeat Violation.
  • Employee changed out water on steam well. He reheated the meatballs. Sliced tomatoes DISCARDED.
